Welcome to FloMotion Physical Therapy. By accessing or using our website and booking services, you agree to these Terms of Use.

Our Services

FloMotion Physical Therapy provides private, cash-pay, in-home physical therapy services in Novato and Marin County, California. All treatment is provided by Zach Siebel, a licensed Doctor of Physical Therapy.

Appointments and Cancellations

  • Appointments must be scheduled in advance.

  • We require 24 hours advance notice for cancellations or rescheduling.

  • Cancellations with less than 24 hours notice or no-shows will be charged a $50 cancellation fee.

  • Repeated late cancellations may result in the need to prepay for future appointments.

Payment

  • All services are due at the time of service.

  • We currently accept Cash, Venmo, and Check.

  • Credit Cards (coming soon).

  • Payments must be made in full at the time of each visit.

Medical Disclaimer

These Terms are governed by the laws of the State of California. Any disputes will be resolved in Marin County, California.
